Privacy policy

Sakura Health LLC ("we," "us") runs this website at sakurahealthdpc.com. This policy explains what information the website collects, how we use it, and the choices you have. We've kept it as plain as we can — if anything is unclear, ask us.

What this policy covers

This policy covers the website, including the Meet & Greet request form. It does not cover your medical records. Once you become a patient, the health information in your record is protected under our Notice of Privacy Practices, which explains how we use and disclose your health information and your rights over it. You can ask for a copy at our office or by contacting us.

Information you give us

When you request a Meet & Greet, the form asks for:

  • Your first and last name
  • Your email address and phone number
  • Your age
  • How you'd like us to contact you (email, phone call, or text message)

We also receive whatever you choose to include when you call, email, or message us.

Please don't send medical details through the website form, email, or social media. We'd rather talk about your health with you directly, at your Meet & Greet or visit.

Information collected automatically

The website itself does not set cookies, and we don't use analytics, advertising pixels, or tools that track you across other websites.

Like any website, our hosting provider (Vercel) automatically processes basic technical information to deliver the pages and keep them secure — such as your IP address, browser type, the pages you request, and when. This information is kept in the hosting provider's logs; we don't use it to identify you.

Third-party services on our pages

A few parts of the site come from other companies. When they load, those companies receive technical information such as your IP address, and their own privacy policies apply:

  • Typeform provides the Meet & Greet form, which loads from Typeform when you open our pages. What you type into the form is sent to and stored by Typeform on our behalf. Typeform may use cookies or similar technologies to make the form work.
  • Google Fonts supplies the typefaces the site uses, which load from Google when you visit.
  • Google Maps loads only if you choose "Show map" in the Location section. Until you do, no request is made to Google Maps.

Our site also links to other websites: the Hint member portal (where members sign in and book visits), our Instagram and Facebook pages, and Google Maps directions. Those sites are run by other companies, and their privacy policies apply once you're there.

How we use your information

  • To respond to your request and schedule your Meet & Greet
  • To contact you the way you asked us to, and to follow up about becoming a member
  • To answer your questions
  • To keep the website running and secure
  • To meet our legal obligations

How we share it

We do not sell your information, and we don't share it for advertising. We share it only:

  • With service providers that help us run the practice and this website — such as Typeform, our email provider, and our hosting provider — who handle it on our behalf
  • When the law requires it, or to protect the rights, safety, or property of our patients, our practice, or others
  • As part of a sale, merger, or transfer of the practice, in which case the new owner must protect it as this policy describes

Email and text messages

If you choose email or text, we'll use it to reach you about your request and your Meet & Greet. Standard message and data rates from your carrier may apply. You can ask us to stop at any time.

Regular email and text messages aren't encrypted and can be intercepted, so please keep sensitive health information out of them.

How long we keep it

We keep Meet & Greet requests only as long as we need them to respond, follow up with you, and keep reasonable business records. If you become a patient, the information becomes part of your medical record and is kept as required by law. You can ask us to delete a request at any time.

Your choices

You can ask us to:

  • Tell you what information we have from your website request
  • Correct it
  • Delete it
  • Stop contacting you

Just contact us, and we'll respond promptly. We may need to confirm your identity first. Requests about your medical record are handled under our Notice of Privacy Practices.

Children

This website is not intended for children under 13, and we don't knowingly collect their information through it. Membership is available for patients 12 and older; for a patient under 18, a parent or guardian should complete the request form. If you believe a child under 13 has sent us information, contact us and we'll delete it.

Security

The site is served over an encrypted connection (HTTPS), and we limit access to your information to the people at Sakura Health LLC who need it. No method of sending or storing information is completely secure, but we take reasonable steps to protect what you share with us.

Changes to this policy

If we change this policy, we'll post the updated version here and change the effective date at the top.
